Behind the ‘A’ Safety Grade: What Methodist Southlake Doesn’t Want You to See

Today, I got an email promoting Methodist Southlake Medical Center proudly announcing that they earned their first-ever “A” Safety Grade from Leapfrog, the national nonprofit that ranks hospitals on patient safety.


The subject line?


“Methodist Southlake earns top marks for hospital safety.”

The reality?


My 11-year-old son was discharged in early sepsis from that very ER, ended up in ICU septic shock within 48 hours, and nearly died.


You know what wasn’t in that email?


  • No mention of the federal EMTALA violation

  • No mention of the hospital’s own peer-reviewed admission that they failed to recognize and treat sepsis

  • And certainly no mention of their ongoing effort to hide critical safety documents in court


Methodist’s Internal Peer Review: Acknowledged Failure


According to a federal CMS investigation (Form 2567, April 2024), Methodist’s own Medical Director conducted a peer review after Nicholas’s discharge and wrote:

“Sepsis was not recognized or treated. The patient was inappropriately discharged.”

Those are not my words. That’s the hospital’s own internal finding, verified by the federal government.


What Methodist Is Doing Instead of Owning It


  • ❌ They’re celebrating safety ratings while refusing to explain how a child ended up in ICU after being sent home from their ER

  • ❌ They’ve shut down every attempt to understand what went wrong, refusing to share internal protocols, timelines, or what staff were trained to do that day

  • ❌ They hide behind “confidentiality” while stonewalling the very family whose child almost died under their care

  • ❌ They deny responsibility for the physician involved, even though their own medical leadership reviewed the care


Meanwhile, they’re emailing parents like me a press release telling us how “safe” they are.


“A” for Safety? Or “A” for Avoiding?


Hospitals don’t earn safety grades by releasing PR statements.


They earn them by facing the hard questions and by releasing the records when something goes wrong or simply doing the right thing.


Instead, Methodist is:

  • Refusing to give documents

  • Dodging accountability

  • And celebrating a rating from Leapfrog like nothing happened


If this is what an “A” hospital looks like… what would a “C” have done?

Final Thought: Trust Is Earned. Not Graded.


My son almost died. Methodist admitted that care failed.

But they’ve spent the last 2 years doing everything they can to keep the truth sealed.

Patient safety isn’t a letter grade. It’s what you do after something goes wrong.

And Methodist Southlake has shown us precisely what that looks like for them.


When someone tells you who they are, listen.
